The entry forms are available in two
formats. One format is MS Word. If you have Microsoft Word as a program
on your computer, please download the MS Word version of the entry form
and open it on your computer. You can then type your team information
into the form and print the file to be sent back to the Tournament Director
at:

If you do not have MS Word on your computer you can
use the PDF link on this page to open a PDF copy of the entry form.
This does; however, require a free program called Adobe Acrobat Reader
that can be downloaded by clicking on this link 
and following the instructions. The entry form can then be filled out
by typing directly on the PDF file. The file can then be printed to
be sent back to the Tournament Director at the above address.
You may use either method but please type your entry
information.
